Community Care City of Kawartha Lakes

Community Care City of Kawartha Lakes is a non-profit charitable organization founded in 1985. Since inception, Community Care has focused on being a leader in the provision and co-ordination of health promotion and community support services. Primary health care, as well as hospice services, were recently added to the agency’s basket of services. The work of Community Care is supported by dedicated volunteers and staff.

Services

Community Care City of Kawartha Lakes is a health and community support agency which provides a range of services designed to promote health for all ages and independence of seniors and special needs clients.

Since inception in 1985, Community Care has steadily expanded its services and scope of care. In the past 26 years, the agency’s basket of services has included Meals on Wheels, transportation, adult day services, assisted living/supportive housing, caregiver support, reassurance, friendly visiting and more recently, services such as foot care, cardiovascular health promotion and chronic disease self-management education.

Services have historically been rooted in the tradition of “neighbours helping neighbours,” delivered by volunteers and staff from multiple sites throughout the City of Kawartha Lakes, often in partnership with other organizations.

The agency also operates a newly established Community Health Centre (CHC) which provides primary health care services through physicians, nurse practitioners, nurses and other allied health professionals.

The Agency supports health promotion and wellness through both its community support services and its health centre services. The Community Health Centre model of care is used because we believe that it is important to think of health as both the treatment of disease as well as its prevention. Through a multi-faceted approach which considers the physical, emotional, mental and social environments of an individual, services address all these areas.
