Accessible Home Health Care© is a full service home health care company. Through our trained professional caregivers we provide medical, non-medical, convalescent care, chronic care, and companion services to patients of all ages from newborn to seniors in their own home!

Skilled Nursing - Our Registered Nurses (RN’s) and Licensed Practical Nurses (LPN’s) provide a full range of skilled nursing services, to include but not limited to:
Initial Assessments
Plan of Care Preparation
Re-assessments as merited to maintain a current plan of care.
Our skilled nursing personnel take required samples for labs tests, provide wound dressing changes, nutritional counseling and medication training

Health Care Assistance Services - Our certified and Licensed Home Health Aides (HHA), Personal Care Aide (PCA), and Certified Nursings Assistants (CNA), provide custodial care and assist with Activities of Daily Living (ADL’s) which include assistance with:
bathing
dressing
walking
medications and personal care depending on state regulations

Specialized HHA’s and CNA’s provide a wide range of care, for clients with Alzheimer’s & Dementia or in a caring for a newborn or special needs children.

Companion Services - Our companions provide non-medical service and keep our clients active and relieved of the loneliness they so often experience. Some of the services provided include:
housekeeping
laundry
meal preparation
transportation

Therapy - Accessible Home Health Care© therapists provide physical, occupational, and speech therapy at home to speed recovery from illness, injury or surgery. This reduces risk and strain of transport to and from a rehabilitation facility.

Durable Medical Equipment - Accessible Home Health Care© has partner firms and can help arrange for home care medical equipment such as:
Crutches
Wheel Chairs
Beds
Walkers
Oxygen

Pharmacy - Accessible Home Health Care© will arrange to fill our client’s prescription and/or pick the prescription up from the pharmacy.
